Registration Form and Liability Waiver

Your $225 registration fee may be paid with
check, money order, or PayPal.

• Though citizens may not sponsor themselves, they may be sponsored by:
  • parent
  • individual adult
  • Legion Post
  • church
  • bank
  • union
  • school or school organization
  • civic organization
• Sponsors are responsible for making sure the student meets all of the ELIGIBILITY requirements
• Sponsors are responsible for making transportation arrangements for students. Many Legion Posts and some schools organize group transportation for a fee. Contact your local school or Legion Post for further information.
• Upon receipt of registration, students will automatically receive an application for the SAMSUNG SCHOLARSHIP. This application will be collected on the Saturday evening of your arrival.
  • Do NOT attach proof of veteran status. If you are chosen as the Illinois winner, we can assist you in obtaining the necessary form DD-214 or equivalent proof of veteran status.
  • School information is REQUIRED in order for the application to be processed.
• While we try to have the registration process complete by May 15, we understand that there will be a few additonal registrations after that date. Please contact the webmaster if you wish to complete late registration.
